If your BIR TIN Card is LOST:
1. Secure an AFFIDAVIT OF LOSS.
Of course, it is actually needed. For legal thing. You can make your own if you want to and have it notarized for FREE in a Public Attorney's Office (PAO) near you. Or just go to PAO, they already have an AoL form there, just fill out the needed information and have it notarized.
If your BIR TIN Card is DAMAGED:
1. Bring/Surrender your Old or Damaged Card.
2. Go to your BIR Revenue District Office (RDO) where your TIN is registered.
Every TIN is registered in a specific RDO. It is where you or your employer pay your taxes. If in doubt, go to any BIR office near you and verify where RDO your TIN is registered. So basically, you cannot process your lost or damage TIN Card in just any BIR office.

3. Ask for BIR FORM 1905 (Application for Registration Information Update) in 1 copy and BIR FORM 0605 (Payment form) 3 copies.
Fill in the needed data. Once done, go to Verification Section give the forms plus a one valid id, they will mark the forms and signed it.
4. Go to any BIR Authorized Agent Banks near you. (LIST HERE)
Now you need to pay Php 100.00 as replacement fee for your new BIR TIN Card. In a bank, ask for a form needed for BIR payments, fill out the needed information wait for your number to be called and pass it to the bank teller together with the 3 copies of BIR FORM 0605 PAYMENT FORM. The teller will give you the official receipt and will return 1 copy of the BIR FORM 0605.
5. Go back to your BIR RDO.
Finally, in the TIN ISSUANCE SECTION, give the official receipt, together with the BIR Forms 1905 and 0605, and your One (1) valid ID. Wait for about another 30 mins. They will call your attention if your card is already printed. Sign it, attached 1x1 colored photo of you and have it laminated. And your done.
